What Is Kefir — And Why Is It So Much More Than Yoghurt?
The fermented food that directly supports your gut, your hormones, and your immune system.

Most people who have heard of kefir think of it as a kind of drinkable yoghurt — something slightly sour, vaguely good for you, sold in a small bottle next to the dairy aisle. And they are not entirely wrong. But that framing undersells it enormously.
Kefir is one of the most potent fermented foods available to us. It contains a diversity of live bacteria and yeasts that most commercial yoghurts simply cannot match. And for the women I work with — many of whom are dealing with digestive discomfort, immune challenges, hormonal imbalances, disrupted sleep, or a nervous system stuck in the on position — it is one of the first foods I talk about.
Not because it is a cure. Because it is a consistent, accessible, and well-evidenced input into systems that govern how you feel every day.
What kefir actually is
The word kefir comes from the Turkish keyif — meaning "feeling good." That is not a coincidence.
Kefir is a fermented drink that originated in the Caucasus Mountains, where it has been made for centuries by fermenting milk with kefir grains. These grains are not grains in the cereal sense — they are small, cauliflower-shaped clusters of bacteria, yeasts, proteins, and polysaccharides that ferment the milk around them, transforming its chemistry in the process.
The result is something meaningfully different from the milk you started with. The lactose is largely broken down — which is why many people who struggle with dairy find they can tolerate kefir well. The proteins are partially pre-digested. And the fermentation produces a vast community of live microorganisms, along with organic acids, B vitamins, vitamin K2, and a unique compound called kefiran, which has its own immune-modulating properties.
Kefir is not just fermented milk. It is a living food — still active when it reaches you, still doing its work in your gut.
Why kefir is not the same as yoghurt
This is the distinction that matters most, and it is worth being precise about it. A standard probiotic yoghurt typically contains two to seven strains of bacteria — most commonly Lactobacillus bulgaricus and Streptococcus thermophilus. Some commercial versions add a few more. They are beneficial, but the diversity is limited.
Kefir, fermented with kefir grains, can contain thirty to fifty-six different strains of bacteria and yeasts — including species that colonise the gut in ways that lactobacillus-only cultures cannot. The fermentation is also more complete, the environment more complex, and the resulting community of organisms more diverse.
Diversity is exactly what the gut microbiome needs. A microbiome with a wide range of bacterial species is associated with better digestion, stronger immune response, more stable mood, and lower systemic inflammation. Kefir supports that diversity in a way that most yoghurts do not.
On labels: look for "live and active cultures" and ideally a specific list of strains. Avoid products that have been heat-treated after fermentation — this kills the beneficial bacteria. Kefir should be refrigerated, not shelf-stable.
Milk kefir and water kefir
Milk kefir is made with dairy milk and kefir grains. It is rich in protein, calcium, B vitamins, and fat-soluble vitamins, and has the full range of probiotic strains associated with kefir's benefits. This is the version most readily available in UK supermarkets and health food shops, and the one I use and recommend most often.
Water kefir is made with sugar water or coconut water and a different type of grain. It contains no dairy, which makes it an option for those avoiding milk — but the microbial community is different, the nutrient profile is different, and the evidence base is not quite as strong. It can be a useful alternative; it is just not identical.
If you tolerate dairy, milk kefir is the more nutritionally complete choice. If you do not, water kefir is worth exploring — and it is worth also looking at other fermented dairy-free options such as coconut yoghurt with live cultures, kimchi, and miso alongside it.
What kefir supports — and why
Digestion
The gut lining relies on a healthy microbiome to maintain its integrity — the tight junctions between cells that prevent undigested particles from passing into the bloodstream. When the microbiome is depleted or imbalanced, this lining becomes more permeable, contributing to the chronic, low-grade inflammation that underlies so many symptoms: bloating, irregular bowel movements, food sensitivities, fatigue after eating.
Kefir's probiotic strains — particularly Lactobacillus kefiri, which is unique to kefir — have been shown to support the restoration of the gut lining, reduce intestinal inflammation, and improve motility. Kefiran, the polysaccharide unique to kefir grains, has additional anti-inflammatory effects on the gut wall.
For those with lactose intolerance: the fermentation process breaks down approximately 99% of the lactose in milk kefir. Many people who cannot tolerate milk or standard yoghurt find they manage kefir well. Start with a small amount and notice how your body responds.
Immune function
Somewhere between 70 and 80 per cent of the immune system is located in and around the gut — in the gut-associated lymphoid tissue and the mucosal lining of the intestinal wall. This is not incidental. The gut is where the body encounters the outside world, and the immune system has evolved to monitor and respond to everything passing through it.
The microbiome is in constant conversation with that immune tissue. A diverse, well-populated microbiome trains the immune system to distinguish between harmful pathogens and harmless substances. When the microbiome is disrupted, that calibration goes wrong, and the immune system can become either under-responsive or over-responsive.
Kefir has been shown to modulate immune response in both directions — reducing excessive inflammatory activity while supporting appropriate immune defence. It also produces bacteriocins: natural antimicrobial compounds that can inhibit the growth of harmful bacteria without disrupting the beneficial microbiome.
A well-fed microbiome is the immune system's most important training ground. Kefir is one of the most direct ways to nourish it.
Hormonal health
Within the gut microbiome there is a specific collection of bacteria collectively known as the estrobolome — the subset of the microbiome responsible for metabolising oestrogen. These bacteria produce an enzyme called beta-glucuronidase, which processes oestrogen in the gut and determines how much is reabsorbed into circulation and how much is excreted.
When the microbiome is healthy and diverse, the estrobolome does this job efficiently — oestrogen levels remain balanced, and the body can clear excess oestrogen effectively. When the microbiome is disrupted, the estrobolome malfunctions: too much oestrogen can be reabsorbed, contributing to symptoms of oestrogen dominance. In the other direction, insufficient microbial activity can leave oestrogen levels too low — relevant for women in perimenopause and beyond.
Supporting the microbiome through fermented foods like kefir is, in a very real physiological sense, supporting hormonal balance. This is not alternative medicine. It is the emerging science of the gut-hormone axis.
The gut-brain connection
Approximately 90% of the body's serotonin is produced in the gut — not the brain. Serotonin is a neurotransmitter involved in mood, sleep regulation, appetite, and the feeling of calm and contentment. Its production depends directly on the health of the gut microbiome and the availability of the right precursors in the diet.
A depleted microbiome produces less serotonin. Less serotonin affects mood, disrupts sleep, and makes the nervous system less resilient under stress. Kefir, by supporting microbial diversity, supports serotonin production — which connects back to everything: how you feel, how you sleep, how you respond to difficulty.
This is the gut-brain axis in practice. And it is why gut health and mental and emotional wellbeing are not separate conversations.

How to choose kefir — what to look for
- Live cultures are non-negotiable. The bottle should state that the product contains live and active cultures, ideally with a strain list.
- Avoid anything that has been pasteurised or heat-treated after fermentation — this kills the bacteria that make kefir worth drinking.
- Whole milk is preferable to low-fat. The fat-soluble vitamins in kefir require fat to be absorbed.
- Organic where possible. The quality of the milk affects the quality of the kefir.
- A short ingredient list is a good sign. Kefir is milk and cultures. If the list is long, look more closely at what has been added.
Making your own kefir with live grains gives you the freshest, most potent version — and once you have the grains, it costs very little. Grains are widely available online and can be reused indefinitely.
How to use kefir in your kitchen
Kefir has a pleasantly tangy flavour that works across a wide range of uses. I use it regularly — both as a drink on its own and as a component in cooking.
- In dressings and dips: whisked with olive oil, lemon, garlic, and herbs, it makes a light, probiotic-rich dressing over salads, roasted vegetables, and grain bowls.
- In smoothies: blended with banana, frozen berries, a little raw honey, and a pinch of cinnamon. The tartness pairs well with fruit and the texture is smooth rather than heavy.
- As a side or accompaniment: simply sliced cucumber, fresh mint, and a generous spoonful of kefir alongside a warm meal. The cold, tangy freshness alongside something warm and spiced is a combination that works every time.
- In overnight oats or chia puddings: use kefir in place of milk or yoghurt for a more complex, gut-supportive base. Add berries, seeds, and a little maple syrup or raw honey.
Kefir is not a supplement. It is food — real, fermented, living food with a history stretching back centuries and a growing body of scientific evidence behind it. Like most things that genuinely support the body, it works best as a consistent presence rather than an occasional intervention.
